Tang Medical Author and Compiler

Sun Simiao

Sun Simiao was a seventh-century Chinese medical author whose two large Qianjin compilations brought prescriptions, diagnosis, acupuncture and moxibustion, dietetics, childbirth, paediatrics, religious therapy, and the conduct of healers into one unusually wide textual project. His secure historical importance rests on those books, not on the many miracle stories later attached to his life.

Traditionally dated 581–682, Sun belonged to the Sui–Tang world of north China. Those dates, his supposed extreme longevity, and stories that emperors repeatedly summoned him come from difficult biographical traditions. Modern scholarship therefore treats “581–682” as conventional and distinguishes the medical compilations from the later image of Sun as recluse, immortal, and “King of Medicine.” 1

Knowledge Worth Gold

Two compilations made a working archive of medieval medicine

The Beiji qianjin yaofang (usually translated as Essential Prescriptions for Urgent Need Worth a Thousand in Gold) was compiled in the mid-seventh century, conventionally around 652. A later thirty-juan supplement, the Qianjin yifang (Supplement to Prescriptions Worth a Thousand in Gold), extended the project. A juan is a textual division often rendered “scroll” or “fascicle,” not a modern chapter of fixed length. 2 3

Compilation was the achievement

Sun did not claim a single discovery. The books selected and organised a large inherited literature of formulas alongside pulse examination, acumoxa (acupuncture and moxibustion), diet, regimen, and treatments for named disorders. Their value lay in making diverse material retrievable while also commenting on how a learned practitioner should judge it. 2

Women and children were placed near the front

Three early juan gather “prescriptions for women,” followed by material on infants and children. This organisation gave reproductive and paediatric care unusual prominence in a learned compilation. It did not, however, supply women’s unmediated voices: the surviving categories and explanations were framed by male textual authors and often centred fertility, pregnancy, childbirth, menstruation, and family continuity. 4

Medical learning crossed religious boundaries

The collections combine classical medical reasoning with Buddhist and Daoist concepts, ritual and demonological treatments, longevity practices, and knowledge associated with travel across Asia. That mixture should not be arranged as a modern story in which “rational” medicine replaced “magic”: in medieval China these approaches coexisted and could be used by the same learned compiler. 2 3

Technique and character were joined

Two opening essays, commonly translated as “On the Professional Practice of the Great Physician” and “The Perfect Integrity of the Great Physician,” insist that extensive learning, diagnostic care, composure, compassion, and restraint belong together. Ethics was presented as part of competent practice, not as a detached oath. 5

Chronology and Evidence

The books are firmer evidence than the heroic life

Sun’s biography has to be reconstructed from texts written for different purposes: remarks attributed to his lifetime, Buddhist and Daoist accounts, official histories compiled later, and the medical works transmitted under his name. These sources preserve history, but they also construct an exemplary recluse and long-lived sage. 1

  1. Traditional 581: the usual birth year. Sources disagree about how to interpret chronological statements concerning Sun’s age, so some scholarship prints question marks beside both life dates rather than treating a 101-year lifespan as established fact.
  2. 581–618, Sui dynasty: later accounts place Sun’s early life in the era that reunited much of China after centuries of political division. He inherited medical canons, recipe collections, religious healing, and state-sponsored scholarship already in circulation; he did not begin from a blank field. 3
  3. 618 onward, Tang dynasty: court institutions and private compilers both organised medical knowledge. The Tang government’s Newly Revised Materia Medica of 659 provides useful context: Sun’s compilation was contemporary with, but not identical to, an imperial effort to collect and standardise drugs. 2
  4. Mid-seventh century: the first Qianjin collection took shape, usually dated to the 650s. Its scale and internal variety record editorial gathering over time more clearly than a single moment of “publication” in the modern sense.
  5. Later seventh century: the Qianjin yifang extended the earlier project. It is conventionally associated with the end of Sun’s life in 682, but neither the traditional death date nor a precise final publication event should bear more certainty than the evidence allows. 1
  6. 1066, Northern Song: the state Office for Editing Medical Texts revised and printed a thirty-juan version of the Beiji qianjin yaofang. Another surviving lineage, the twenty-juan Sun zhenren qianjin fang, differs substantially in order and wording. The text readers encounter is therefore a transmission history, not an untouched seventh-century manuscript. 3 7

It is a primary source for later reception and textual form, not direct evidence for every word Sun wrote nine centuries earlier. 8

Practice in Context

The formularies preserve a plural and materially demanding medicine

“Prescription” here means more than a list of ingredients. Entries could specify selection, processing, combinations, quantities, timing, route of administration, diet, and conduct. Other parts of the compilation concern pulse signs, needling and burning moxa, exercises, massage, incantation, talismans, and the cultivation of life (yangsheng). “Urgent need” in the title signals the practical value of having remedies available; it does not make the book an ancestor of a modern emergency department. 2

The text’s illness terms belong to medieval systems of the body. Categories such as “wind,” “cold damage,” “foot qi,” the “seven openings,” depletion, or demonic affliction cannot simply be replaced by a current diagnosis. Some symptom clusters can be compared with conditions recognised today, but a one-to-one translation erases the explanations that guided historical treatment. The same caution applies to “diet therapy”: foods, drugs, seasonal regimen, and self-cultivation formed overlapping rather than neatly separate domains. 2

Nor were “natural” remedies necessarily gentle. Medieval pharmacy included deliberately potent minerals, plants, and animal substances; practitioners debated how processing and dosage could turn a dangerous substance into an effective medicine. Ingredient names, identities, preparation, and textual readings have also changed. The Qianjin recipes are evidence for historical practice and belief, not instructions for self-treatment. 6

Patients and Categories

Prominence did not mean equality of perspective

Sun explained the decision to begin with women and children as “venerating the root”: reproduction and the continuation of families made their care foundational to the social order. The arrangement is historically important, but it was not modern advocacy for women’s health. Women’s bodies were often described as especially difficult to know, and menstruation became a key sign through which male authors interpreted fertility and illness. 4

Earlier Chinese writings already contained recipes for childbirth and reproductive problems. Sun’s contribution was therefore not the “invention of gynaecology.” His work gathered, theorised, and prominently organised a developing literature of “prescriptions for women.” Historians differ over when a fully gendered model of the body emerged between the Han and Song periods; the Qianjin sections are central evidence in that debate, not a reason to declare one uncontested first. 4

The paediatric sections similarly make infants and children visible within learned medicine, while revealing adult assumptions about vulnerability, growth, diagnosis, and household care. A compilation can preserve knowledge used around bedsides without identifying all the caregivers, birth attendants, family members, or patients through whom that knowledge circulated.

The Great Physician

An ethic of disciplined attention, not a “Chinese Hippocratic Oath”

In the essay Dayi jingcheng—translated variously as “The Perfect Integrity,” “Absolute Sincerity,” or “Great Physician’s Sincerity”—the healer must settle the mind, respond compassionately to suffering, and disregard distinctions of wealth, rank, age, beauty, enmity, kinship, origin, and intelligence when caring for a patient. The passage also warns against self-promotion and pursuit of wealth. 5

Its compassion toward living beings and hesitation over killing animals for drugs have clear Buddhist resonances, while its ideal practitioner also draws on Daoist self-cultivation, classical learning, and social obligations often described as Confucian. Assigning the passage to only one tradition flattens the intellectual world in which Sun compiled it. 5 3

Calling this a “Chinese Hippocratic Oath” is a modern comparison, not its historical identity. The essay was not sworn in the institutional setting of a Greek oath, and its moral psychology, cosmology, religious vocabulary, and expectations of learning are its own. The comparison may signal importance, but it should not replace the text’s Chinese title or Tang context.

Transmission and Reputation

Editors, printers, worshippers, and historians made several Sun Simiaos

The Qianjin books mattered after the Tang because people kept selecting, copying, quoting, correcting, and printing them. Song editors did not merely reproduce an original: they changed order and wording, while other textual lineages continued beside the state edition. Variants preserved in later books can help scholars reconstruct earlier readings. What survives is both Sun’s project and centuries of editorial labour. 7

A second afterlife transformed the learned author into Sun Zhenren, “Perfected Sun,” and Yaowang, “King of Medicine” or “King of Medicinals.” Temples, portraits, miracle tales, and claims of extraordinary longevity joined medical abundance to religious power and moral exemplarity. Those traditions are historically important evidence of commemoration; they cannot verify stories about whom Sun cured or which emperor sought his service. 1 3

Sun’s durable importance is therefore collective as well as individual. He assembled earlier learning; patients and unnamed practitioners supplied the world from which recipes came; later scholars stabilised and altered the books; and religious communities made their author a healing sage. Separating these layers gives him a richer history than the claim that one timeless genius created “Chinese medicine.”
