How Medical Knowledge Travels

Medical knowledge has always moved: between teachers and pupils, languages and scripts, courts and markets, households and hospitals, manuscript pages and printed books. Movement did not merely distribute ideas already complete. It selected, reordered, renamed, illustrated, criticised, and sometimes misunderstood them.

Translation and transmission are forms of knowledge-making. To follow a medical work across time is therefore to study a chain of authors, translators, copyists, practitioners, patrons, printers, collectors, and readers—not the uninterrupted journey of a fixed original.

Movement and Change

Knowledge does not travel without being transformed

A remedy, anatomical claim, or diagnostic category acquires meaning inside a particular language and practice. Moving it requires choices about equivalence, explanation, and use.

Translation is the most visible of those choices. A translator must decide whether an unfamiliar term names a substance, body part, process, disease, or analogy. Sometimes no exact counterpart exists. Transliteration can preserve a foreign word but leave it obscure; substitution can make it familiar while changing its range. When Islamic scholars rendered Galen and Hippocrates from Greek into Arabic, they did not merely copy: they ordered and systematised a vast, sometimes inconsistent Greco-Roman literature so that it could be taught. When Juan Badiano rendered Nahuatl plant names into Latin in 1552, the Nahuatl original has not survived, so the choices of that encounter are now visible only in the Latin. Later readers may then treat the translated term as if it had always been stable.

Material form changes reading too. A long roll encourages a different kind of navigation from a codex with numbered books, a printed edition with indexes, or a searchable digital image. Headings, diagrams, marginal notes, and page layouts determine what can be found quickly and what appears central. The 1482–83 Venice printing of the Latin Canon, set in double columns on royal-size paper, made the work navigable for a university classroom in a way that no single manuscript could be. The history of medical ideas is inseparable from the history of the objects that carried them.

Movement can also fail. Works disappear, remain untranslated, circulate only in fragments, or reach readers who reject them. What survives is not necessarily what was most representative or effective. It may instead reflect the resources of a court, monastery, university, colonial administration, collector, or modern archive. The Badianus Codex illustrates both preservation and loss: it saved 185 illustrated remedies, but the Nahuatl text that was translated into Latin no longer exists.

A Chronology of Routes

Milestones in the movement of medical texts

The sequence matters: each step changed who could read a work, in what language, in what material form, and with what authority.

  1. c. 1550 BCE: The Ebers Papyrus compiles Egyptian remedies in a single manuscript.
  2. c. 78 CE: Dioscorides writes De Materia Medica, which will anchor pharmacological reference for about fifteen centuries.
  3. First centuries CE: The Charaka Samhita takes shape as a layered Sanskrit recension; its chronology remains debated.
  4. 8th–10th centuries: Greek medical authorities are translated into Arabic and reorganised into encyclopedias and summaries.
  5. c. 1025: Ibn Sina completes the Canon of Medicine, worked on over years as he moved from Gurgan to Rayy to Hamadan.
  6. Late 12th century: Gerard of Cremona translates the Canon from Arabic into Latin in Toledo.
  7. 1482–83: The Latin Canon is printed in Venice in five parts, among the earliest printed editions of the work.
  8. 1527: Andrea Alpago's revised Latin version of the Canon, with an Arabic-Latin glossary of terms, is published posthumously by his nephew Paolo.
  9. 1552: The Badianus Codex records Nahua remedies in Latin at the Colegio de Santa Cruz de Tlatelolco.
  10. 1578: Li Shizhen completes the draft of the Bencao Gangmu.
  11. 1590s–1603: The first Jinling edition of the Bencao Gangmu appears after Li's death; a widely influential 1603 reprint becomes the basis for many later editions.

Four Textual Journeys

Composite works became authoritative in different ways

The Charaka Samhita grew through recension

The Sanskrit medical work associated with Charaka presents itself through a lineage — the teacher Atreya, the pupil Agnivesha, a recension by Charaka — and later tradition credits Dridhabala with restoring missing portions. Historians date its layers by language, cross-reference, commentary, and manuscript comparison, but the chronology remains debated. Its durability rests on repeated acts of reconstruction: commentary made the work teachable, copyists selected among variant readings, and printed Sanskrit editions and an English translation associated with Avinash Chandra Kaviratna began appearing in Calcutta in the 1890s. Each stage is an argument about what the work is, not a transparent window onto an original.

The Canon crossed scholarly languages

Ibn Sina (980–1037) composed the Canon over a lengthy period as he moved from Gurgan in northern Iran to Rayy and then Hamadan, where it was completed. Gerard of Cremona's twelfth-century Latin translation, made in Toledo, carried it into European teaching; between 1500 and 1674 some sixty editions of part or all of the work were printed in Europe, mostly for university medical training. Commentaries by Ibn Jumay' (d. 1198), Ibn al-Nafis, al-Amuli, and 'Ali al-Jilani (d. 1609) — the last reporting more than thirty years of work — made several reading traditions from one work. No complete modern European-language translation exists, so the Canon is still read through partial translations and the medieval Latin. European reception was not simply the recovery of Greek medicine: readers encountered a synthesis developed within Arabic scholarly networks.

The Bencao Gangmu joined compilation to correction

Li Shizhen (1518–1593) completed a draft in 1578 but did not live to see the work in circulation. The first Jinling edition appeared in the 1590s with members of his family involved in proofreading and illustration; a widely influential 1603 reprint became the basis for many later editions. The compendium's fifty-two juan and 1,892 entries joined nomenclature, natural history, properties, preparations, and prescriptions. Every reprint could change layout, images, and errors, and modern translations — such as Paul U. Unschuld's nine-volume English edition (University of California Press, 2021–) — render Ming categories into new terminologies that do not map perfectly onto the original.

The Badianus Codex records an unequal encounter

Created in 1552 at the Colegio de Santa Cruz de Tlatelolco in Mexico City, the manuscript — Libellus de medicinalibus Indorum herbis — presents Nahua plant and healing knowledge through the expertise of the physician Martín de la Cruz, the Latin translation of Juan Badiano of Xochimilco, and 185 colour illustrations. Prepared at the request of Francisco de Mendoza, son of the first viceroy of New Spain, it was sent to Spain and was probably intended as a gift for Emperor Charles V. It passed from the Escorial to the collection of a royal pharmacist, then to the Barberini library, entered the Vatican Library in 1902, and was returned to Mexico in 1991; it is now held by the National Library of Anthropology and History. The Nahuatl original no longer survives, so the codex preserves the encounter as much as the knowledge.

Institutions and Power

Routes are created by people with resources

Texts move because institutions pay for copying, teaching, collecting, printing, and preservation. Those investments establish some forms of knowledge as canonical while leaving others difficult to trace.

Courts and religious foundations supported scholars and libraries; book markets connected copyists with buyers; universities turned selected works into curricula — the printed Canon was aimed at university medical training for nearly two centuries; printers gambled on audiences, as the Venetian printers of the 1482–83 Latin Canon did; empires gathered plants, manuscripts, and information, as the Colegio de Santa Cruz de Tlatelolco gathered Nahua remedies for a Spanish court; museums and libraries later assigned provenance and catalogue terms. Each setting opened routes while imposing boundaries.

Authority could increase with distance. A translated book associated with a famous author might appear more coherent than the varied practices from which it emerged. Conversely, practical knowledge supplied by healers, cultivators, merchants, patients, and interpreters could lose its makers' names when absorbed into a learned compilation or colonial collection.

This is why “influence” is not enough as an explanation. Historians must ask what actually moved, through whose labour, in which material form, and for what audience. A text listed in a library does not prove that it was widely read; a translation does not prove agreement; a surviving recipe does not prove its routine use. The 1991 return of the Badianus Codex to Mexico shows that these histories can be partially reversed — custody changed, but the lost Nahuatl wording could not be recovered.

A Method for Reading

Follow the transformations, not just the route

Identify the version

Ask which manuscript, edition, translation, image, or catalogue record supports a claim. Dating the supposed original is not enough when the surviving witness was made for a later audience.

Look for mediators

Translators, scribes, illustrators, editors, teachers, merchants, and collectors are historical actors. Their decisions explain why knowledge arrived in a particular form.

Separate presence from acceptance

A work can be owned without being read, taught without being followed, or translated in order to criticise it. Circulation reveals opportunity, not automatic influence.

Recover unequal exchanges

When knowledge enters an archive through conquest, extraction, or colonial administration, preservation and dispossession may be part of the same history.

Legacy

Transmission is part of the content

Medical knowledge survives because successive communities find ways to make it useful. They reorganise it for teaching, translate it into available concepts, compare it with practice, and preserve it in new media. Continuity is therefore an achievement rather than a passive condition.

That insight changes how medical traditions are compared. It replaces a simple map of isolated civilisations with histories of contact, asymmetry, adaptation, and refusal. It also prevents modern readers from treating a present edition as a transparent window onto an ancient or early modern moment. What we read is the accumulated result of travel.
